A salary slip (also called a payslip or wage slip) tells a worker three things: what they earned, what was cut and why, and what reached them. Here is a format you can follow, a filled sample, and every line in plain words.

ESI is medical cover. The worker gives 0.75% of pay and the employer 3.25%, for people paid ₹21,000 a month or lessverify. Rakesh is paid ₹22,000, so nothing is cut. For a worker who is covered, it is one more line under "Cut from salary".
Tax on salary (TDS) is cut only when the income of the whole year crosses the tax limit. Under the new tax regime there is no tax up to ₹12 lakh of taxable incomeverify. When tax is due, it shows as "Tax (TDS)" under the cuts.
Copy these lines into your own slip, in this order. The last column shows what the sample above has in that place.
The Code on Wages asks an employer to give a wage slip on or before the day wages are paidverify. The exact form and its columns are set in the rules, and the rules of your state may ask for moreverify. This page is a working format that a worker can read. It is not a government form. Ask your CA which form your state asks for.
The slip keeps the same shape for every worker. Only the "Earned" line is worked out differently. The cuts and the amount paid sit below it in the same way.
